To book the appointment (online)
No upload is needed. Just keep these details handy while filling our booking form:
- Passport – number, issue date, issue place and expiry date (exactly as printed).
- Full name – given name and surname exactly as in the passport.
- Date of birth, gender and marital status.
- Country travelling to and visa type (Work / Family).
- Job title as written on your visa or offer letter.
- Phone number and email address – the slip is sent to your email.
- Medical city and your preferred appointment date.
To carry on the medical day

Important tips
- Name and passport number must match exactly. A mismatch can cause trouble at the centre or during visa stamping.
- Reach on time. Arrive 15–30 minutes early. Late arrivals may be asked to reschedule.
- Fasting: follow the instructions on your slip or given by the centre.
- Women: inform the centre if you are pregnant before any X-ray.
- Go in person. The passport holder must attend; identity is verified with photo and biometrics.
- Never share OTPs or passwords with anyone claiming to “speed up” your result.
Common questions
Do I need to upload documents to book?
No. Eazy GAMCA Appointment only needs the details typed into the booking form. You carry the original documents to the medical centre on the appointment day.
Is a photocopy of the passport enough at the medical centre?
No. The original passport is required for identity verification. Carry a photocopy as well, as many centres ask for one.
